– ثم ادخل الرقم القومي المكون من 14 رقم الخاص بالشخص ذو الإعاقة ليتم الاستعلام.
– انقر على أيقونة بحث وسوف تظهر لك النتيجة

هناك بعض الفئات المستحقة لاستخراج بطاقة الخدمات المتكاملة من أصحاب الإعاقات، وهم: الشلل الدماغي، الشلل الرباعي، الشلل النصفي الطولي أو السفلي، شلل الأطفال الشديد، الإعاقات المتعددة، البتر متعدد الأطراف أو في طرف واحد (يد أو قدم)، كف البصر، فئات فقد السمع التام، الحالات المتقدمة من ضمور العضلات، الإعاقات الذهنية الشديدة، ذوي القزامة “140 سم فأقل بعد سن البلوغ”، بالإضافة إلى اضطراب طيف التوحد.
