ـ إيصال تأمين التابلت الأصلى بعد سداد مبلغ التأمين فى مكتب البريد قدره 105 جنيهات و2 صورة من الإيصال.
ـ إقرار تسليم التابلت الذي يوقعه ولي الأمر.
ويوقع ولي الأمر والطالب بالصف الأول الثانوي على إقرار استلام جهاز التابلت، وأن يتم تسليمه مرة أخري بعد الانتهاء من مرحلة الثانوية العامة، وأن يتم الحفاظ عليه دون العبث أو التلف أو الفقد.
وينص نموذج إقرار استلام جهاز تابلت (الطالب، وولى أمره) الثانوي العام التابعة للإدارة التعليمية:
بأن نجلى قد تسلم جهاز حاسب آلي لوحي (تابلت) مسلسل رقم IMEI ، والتزم أنا ونجلى بالمحافظة عليه من العبث أو التلف أو الفقد، وفى حالة تعرضه للتلف أو الفقد أكون ملتزما بسداد قيمة الجهاز كاملة، شاملة كافة النفقات التي تحملتها الوزارة، كما التزم بتكاليف صيانته في حالة حدوث أي أعطال يكون نجلي قد تسبب فيها على نفقتي الخاصة، وذلك بمراكز الصيانة المعتمدة التابعة الشركة سامسونج بجمهورية مصر العربية، وأتعهد بعدم الإصلاح خارج مراكز صيانة الوكيل المعتمد.
وأتعهد بتسليم جهاز التابلت متى انتهى قيد نجلي بالمرحلة الثانوية بأي سبب من أسباب انتهاء القيد سواء بالنجاح أو غيره، مع التوقيع وكتابة التاريخ وبيانات الطالب والرقم القومي ولى أمر الطالب والتوقيع والرقم القومي.
