كشفت وزارة الصحة عن رابط إصدار بطاقة الخدمات المتكاملة 2023، ونستعرض في التقرير التالي الخطوات والمستندات وآلية التقديم والمتابعة للحصول على كارت الخدمات المتكاملة.
مستندات بطاقة الخدمات المتكاملة
ودشنت وزارة الصحة موقع خاص لإصدار التقرير الطبي المميكن لذوي الاعاقة حيث يدخل ذوي الاحتياجات الخاصة لتسجيل كافة البيانات المطلوبة على الموقع وتشمل:
أولًا: الرقم القومي، وبالنسبة للأطفال يتواجد الرقم القومي في شهادات الميلاد المميكنة.
ثانيًا: كتابة الأسم بشكل صحيح في الخانات المعدة على الموقع.
ثالثًا: كتابة المحافظة، والعنوان على ألا يتعدى العنوان 14 حرف.
رابعًا: اختيار نوع الإعاقة، وتشمل ( أمراض الدم، الإعاقة البصرية، الإعاقة السمع بصرية، الإعاقة السمعية، الإعاقة الذهنية، الإعاقة الحركية، اضطراب طيف التوحد، القزامة، الإعاقات المتعددة)
خامسًا: الحالة الاجتماعية
سادسًا: الوظيفة
سابعًا: الهاتف، ويجب إضافة رقم هاتف محمول لا يفقده مقدم الطلب حيث يعد وسيلة التواصل خلال مراحل إنتاج بطاقة الخدمات المتكاملة.
وللاستعلام عن نتيجة التقرير، يتم إدخال الرقم القومي، بالإضافة إلى الهاتف المحمول الذي تم التسجيل به ليظهر الموقع نتيجة الكشف الطبي.
خطوات التسجيل الصحيحة
أولًا: سجل طلبك من خلال الموقع الإلكتروني، وتأكد من تسجيل البيانات بدقه لأنها مسئولية قانونية وهيتوقف عليها قبول طلبك أو رفضه. تأكد من أن الملفات المرفقه واضحه ومرفقة بشكل سليم
ثانيًا:بعد ملء بياناتك الشخصية يجب ملء إستمارة البيانات الأساسية (مجموعة أسئلة) وتأكد من الإجابة بمنتهى الدقة.
ثالثًا: بعد التسجيل هيظهرلك رقم الطلب الخاص بك – رجاءا إحتفظ به. ملحوظة هامة: ليس معنى أنك قمت بالتسجيل أنه تم قبول طلبك ده أول خطوة لدراسة حالتك ولازم تكمل باقي الخطوات.
رابعًا: قم بالإستعلام عن الطلب من خلال أيقونة الإستعلام عن الطلبات بالبوابة الإلكترونية
خامسًا: في حالة قبول الطلب قم بحجز موعد من خلال أيقونه الحجز بالبوابة وبعدها توجه لمكتب التأهيل الي قمت بإختيارة في الموعد المحدد ومعك كل الأوراق والمرفقات ليتم تسجيل بيانات التقييم الوظائفي الخاص بك.
سادسًا: بناء على الخطوات والبيانات التي تم إستيفائها سيتم تحديد درجة ونوع الإعاقة وإن كنت تستحق الحصول على بطاقة الخدمات المتكاملة للأشخاص ذوى الإعاقة.

وأكد مصدر مطلع بوزارة الصحة أن طلبات المواطنين يتم تحويلها إلى وزارة التضامن الاجتماعي بعد انتهاء الإجراءات الطبية المطلوبة، لتقوم بإصدار بطاقة الخدمات المتكاملة، ونؤكد حرص وزارتي الصحة والسكان، والتضامن الاجتماعي والجهات المعنية على تسهيل حصول ذوي الإعاقة على كارت الخدمات المتكاملة.
